The office is hereby requesting an opportunity to inspect or obtain copies of public records of the following:

1. All complaints regarding the condition of the sidewalk on the south side of Upas Street between 30th St. and Grim Ave. made in 2017.
2. All complaints regarding the condition of the sidewalk on the south side of Upas Street between 30th St. and Grim Ave. made from 2010 through 2016.
3. All work orders or records of repair to the sidewalk on the south side of Upas Street between 301h St. and Grim Ave. made in 2017.
4. All work orders or records of repair to the sidewalk on the south side of Upas Street between 30th St. and Grim Ave. made from 2010 through 2016.
5. All photographs of the sidewalk on the south side of Upas Street between 30th St. and Grim Ave taken in 2017 in connection with either a complaint regarding the condition of the sidewalk or a repair to the sidewalk.
6. All photographs of the sidewalk on the south side of Upas Street between 301h St. and Grim Ave from 2010 through 2016 in connection with either a complaint regarding the condition of the sidewalk or a repair to the sidewalk.

While these requests include the entire block, for your assistance we are specifically interested in the area east of the alley that separates 30th Street and Grim Ave.

7. All complaints regarding the lighting of the sidewalk on the south side of Upas Street between 30th St. and Grim Ave. made in 2017.
8. All work orders or records of repair to the lighting from the light 165120 on light pole P231678 which is located on the south side of Upas Street in the alley between 30th St. and Grim Ave. made in 2017. (See Photos attached)
